Community Resources & Programs Directory
Health & Social Services
South Wasco Community Health Worker and Bridges to Health:
SWA does not provide health or social services. This listing points you to the program that does.
Finding it hard to access resources and services on your own? A Community Health Worker (CHW) can inform, connect, and guide you through complicated systems so you get the help you need. CHWs are trusted members of the communities they serve. They build relationships with community members, local social service agencies, and healthcare providers, and they help make those systems work better for the people they serve. South Wasco has a dedicated CHW, who started in 2024 through a partnership between South Wasco Alliance and the Columbia Gorge Health Council.
Your CHW is part of Bridges to Health (B2H), a program of the Columbia Gorge Health Council that has served Hood River and Wasco counties since 2016. B2H helps community members who face barriers to health and wellbeing, including with health coverage and appointments, food, medications, housing and utility help, and other basic needs, both inside and outside the healthcare system. The program is led by people with lived experience.
